1. Form is created in the Online Portal
Once a form has been created and published in your Online Portal, the form will be assigned to a Client's appointment or Client Profile, depending on your Form Settings.
2. Clients will receive an SMS or Email to fill out a Form
Once a form has be assigned to a Client's appointment or Client Profile, the client will receive an SMS or Email to fill out the form.
3. Clients fill out Forms through Online or in the Booking App
The Client can fill out the form Online or in the Booking App. The form will open in the Form Viewer:
4. The Form Icon will display as green (completed) or orange (not completed) on the Appointment Block
If a client has completed and submitted a Form, the Form Icon on the Appointment Block will display as green (completed)
If a client has not completed and submitted a Form, the Form Icon on the Appointment Block will display as orange (not completed)
5. Form is available in appointment details
If a form is assigned to all services or a specific service, it will be assigned to the clients appointment.
You also have the option to assign a form to an appointment.
Form is available in Client Profile > Forms
Once a client has filled out a Form, you can view it in the Appointment Details screen or in their Client Profile in the Online Calendar.
You can view:
- Incomplete forms (in order of when the form was sent to the customer)
- Completed forms (in order of when the form was completed)
