How do I create a Form that can be sent to a parent to sign?

Instructions:

Include an optional Parent Signature section in your form. When a child who is under 18 goes to book their appointment or once the appointment is booked, they/their parent will be able to fill out the form and add in their signature. Any other customer who is 18+ will not be required to fill out that section to submit the form since the Parent Signature section is optional!

If you need to send the form to the client, you are able to send a form directly to the parent, with the direct Form URL!

Our suggestion:

You have the option of adding the parents email or phone number on the client profile of the child and manually assigning the form to the Client Profile/Appointment - This way, when you assign the form, you have the option to send it to the parents SMS/Email, so the parent can fill it out themselves.
